Description
Palmar dissection exposes the hypothenar compartment with the flexor digiti minimi brevis (flexor digiti minimi) highlighted beneath the removed skin and superficial fascia. Its slender belly lies on the ulnar side of the palm, lateral to the abductor digiti minimi and superficial to the opponens digiti minimi, then narrows into a tendon that runs distally toward the base of the proximal phalanx of the little finger. Deep and radial to this muscle, the hook of the hamate and the flexor retinaculum form the proximal attachment area, while the fifth metacarpal and phalanges provide bony orientation for the metacarpophalangeal joint mechanics. Small but specific. Palmar emphasis on this muscle matters when teaching or planning around ulnar-sided hand symptoms, because the hypothenar muscles sit immediately adjacent to the ulnar nerve and artery as they pass through Guyon canal, and focal compression can produce sensory loss in the little finger with hypothenar weakness or atrophy. Hand surgeons also use these landmarks when approaching hook of hamate fractures, ulnar artery thrombosis (hypothenar hammer syndrome), or when distinguishing intrinsic muscle deficits from flexor digitorum profundus weakness during an ulnar neuropathy exam. Clear spatial relationships reduce diagnostic ambiguity.
