Description
Optic nerve fascicles occupy the center of this sectional view, with densely packed retinal ganglion cell axons arranged in bundles separated by delicate septa. A meningeal sleeve surrounds the nerve, with pia mater closely applied to the nerve surface, arachnoid trabeculae outlining the subarachnoid space, and an outer dural sheath forming the thickest concentric layer. Small-caliber vessels course within the sheath and along the pial surface, reflecting the vascular supply that accompanies the intraorbital nerve and communicates with branches of the ophthalmic artery and central retinal circulation. Subarachnoid continuity between the optic nerve sheath and the intracranial compartment makes this anatomy clinically specific: raised intracranial pressure is transmitted to the perioptic subarachnoid space and contributes to papilledema by impeding axoplasmic flow at the optic disc. The meningeal layers also explain why optic nerve sheath fenestration targets the dural and arachnoid coverings rather than the nerve proper, and why inflammatory processes such as optic neuritis can produce sheath enhancement and perineuritis patterns on orbital MRI. Boundaries are unambiguous. The concentric sheaths and intervening spaces are separated by lighting that helps distinguish true subarachnoid space from the more compact dural layer.
