Description
Encircling the palpebral fissure, the orbicularis oculi forms a continuous sphincter with a thicker orbital part lying more peripheral on the bony rim and a thinner palpebral part spanning the upper and lower eyelids. Medially, fibers converge toward the medial canthus over the lacrimal sac region, while laterally they sweep to the lateral canthus on the zygomatic process of the frontal bone. A semi-transparent craniofacial framework provides orientation, including the frontal bone and zygomatic bone outlining the orbit, the nasal bones at the midline, and the maxilla and mandible with the dentition visible inferiorly. Cervical vertebrae and the hyoid bone appear in the neck for additional head and neck context. Clear landmarks. Orbicularis oculi anatomy matters because its orbital fibers generate forceful eyelid closure and its palpebral fibers support blinking, so subtle differences in fiber distribution help explain both function and symptom patterns. Spasm in these fibers underlies benign essential blepharospasm, and targeted chemodenervation with botulinum toxin is planned around the eyelid margins and canthal regions to reduce spasm while limiting exposure keratopathy. Injury to the temporal and zygomatic branches of the facial nerve (CN VII), whether after parotid surgery or facial trauma, weakens eyelid closure and predisposes to lagophthalmos, a practical correlation when teaching facial nerve testing at the bedside. Ophthalmology and head and neck anatomy courses can use this anterior view to teach surface anatomy of the eyelids, canthi, and the orbital rim, and to pair muscle function with the underlying skull.
