Description
Seen from a medial angle in a male forearm, the palmaris longus arises from the medial epicondyle of the humerus and runs distally as a slender fusiform belly that becomes a long superficial tendon in the mid-forearm. Its tendon lies anterior to the flexor digitorum superficialis and typically courses between flexor carpi radialis laterally and flexor carpi ulnaris medially as it approaches the wrist. With the forearm slightly pronated, the anterior compartment flexor mass is prominent while the ulnar border and medial intermuscular septa read clearly. Small details matter here. Palmaris longus is infamous for variability, absent unilaterally or bilaterally in a substantial fraction of adults, and this medial perspective helps teach how to identify it clinically by its midline tendon when the patient opposes the thumb and flexes the wrist. Surgeons often harvest the palmaris longus tendon for grafting (for example, opponensplasty, ligament reconstruction, or tendon transfers), so its relationship to the median nerve in the distal forearm and to the flexor retinaculum and palmar aponeurosis at the wrist has immediate procedural relevance. A hypertrophied or anomalous palmaris longus (including a reversed muscle belly) can contribute to volar forearm pain or median nerve irritation, a differential point that gets missed when only textbook anterior views are used.
