Description
Running along the plantar aspect from the medial tubercle of the calcaneus, the plantar aponeurosis is shown as a dense band that courses anteriorly beneath the longitudinal arch and fans distally toward the bases of the proximal phalanges. A medial perspective places the calcaneus and talus posteriorly, with the navicular and medial cuneiform superior to the central band as it passes into the forefoot. Metatarsals I to V and the phalanges form the distal framework, while articular cartilage caps the joint surfaces at the subtalar region, midtarsal joints, and metatarsophalangeal joints. A thin skin silhouette preserves external contour. Clinically, this view matters because plantar fasciitis most often originates at the calcaneal attachment on the medial tubercle, and pain localizes where the aponeurosis is tensioned during terminal stance and toe-off (the windlass mechanism as the hallux dorsiflexes). Medial visualization also clarifies why a calcaneal spur is an associated radiographic finding rather than the primary pain generator, and it helps distinguish aponeurotic pathology from tibialis posterior tendinopathy or spring ligament injury that can also present with medial foot pain and acquired flatfoot. A clean side view makes the longitudinal arch relationship obvious.
