Medially along the sole, the plantar aponeurosis (plantar fascia) forms a thick fibrous sheet spanning from the medial process of the calcaneal tuberosity toward the forefoot, where the central band fans distally into digital slips at the level of the metatarsophalangeal joints. From this angle the longitudinal arch is defined by the fascia’s inferior course beneath the calcaneus, navicular, and medial cuneiform, with the hallux side lying superior and medial relative to the lateral plantar compartments. Superficial skin and subcutaneous fat pad sit plantar to the fascia proximally, while the intrinsic flexor layer and plantar neurovascular bundles lie deep to it. Plantar fasciitis most often localizes near the calcaneal origin, so a medial perspective that emphasizes the proximal central band helps when teaching palpation landmarks and correlating heel pain with fascial tension during toe dorsiflexion (windlass mechanism). In surgical planning, orientation of the medial band matters because partial plantar fasciotomy is commonly performed from the medial side to reduce strain while attempting to preserve lateral support and avoid destabilizing the arch. Heel spur discussions also benefit from this view, since enthesophytes arise at the calcaneal attachment rather than within the fascia itself. A common exam finding.
