Description
Plantar anatomy is presented from the sole with the plantar aponeurosis (plantar fascia) spread as a thick, triangular sheet extending distally from the medial process of the calcaneal tuberosity toward the toes. Its central component fans anteriorly over the plantar aspect of the metatarsals and divides into longitudinal digital bands that continue toward the bases of the proximal phalanges, lying superficial to the flexor tendons and the intrinsic plantar muscles. Medially, the fascia blends with the thinner fascia over abductor hallucis, while laterally it continues toward the region of abductor digiti minimi. Dense fibrous tissue dominates. This plantar view matters because the plantar aponeurosis is the primary superficial restraint of the longitudinal arch, and its proximal attachment at the calcaneus is the pain generator in plantar fasciitis and plantar fasciopathy. The depicted distal bands help explain why heel pain can coexist with forefoot symptoms, and why limited first metatarsophalangeal dorsiflexion can increase tensile load along the medial band during gait. It also frames the surgical anatomy of partial plantar fasciotomy and the risks of over-release leading to lateral column pain and arch instability.
