Description

Oriented on the plantar aspect of the male foot, the plantar metatarsal veins run in the intermetatarsal spaces along the plantar surfaces of the metatarsals and deep to the plantar aponeurosis, converging proximally to form the deep plantar venous arch. Distally, tributaries arise from the plantar digital veins near the bases of the toes and course proximally toward the tarsometatarsal region. The deep venous arch lies anterior to the calcaneus and posterior to the metatarsal heads, where it parallels the deep plantar arterial arch and maintains close relationships to the plantar interossei and the tendons of flexor hallucis longus and flexor digitorum longus. Arteries are rendered in red for orientation, with the posterior tibial artery and its plantar branches accompanying the venae comitantes before venous return continues proximally toward the tibioperoneal system. Plantar venous anatomy matters when you are thinking about deep outflow pathways that are not obvious on surface inspection, since the plantar metatarsal veins channel blood from the toe pulp and metatarsal heads into the deep plantar venous arch that empties into the posterior tibial veins. In diabetic foot infection, plantar space abscess, or transmetatarsal and ray amputations, this venous network sits in the same corridor as the deep plantar arterial arch and adjacent neurovascular bundles, so dissection and hemostasis demand a clear mental map. The view also supports interpretation of duplex ultrasound or MR angiography when distinguishing deep plantar flow from the superficial venous drainage of the sole.
