Description
Oriented from a dorsal perspective, the foot is rendered with emphasis on the plantar metatarsal veins coursing deep to the metatarsals along the plantar interosseous spaces, draining blood from the plantar digital territory toward the proximal forefoot. Distally, veins arise from the plantar aspects of the toes and communicate around the metatarsal heads with the dorsal venous network, which lies more superficial on the dorsum of the foot. Proximally, the collecting channels converge toward the tarsus and continue into the deep venous pathways of the leg, in close relationship to the corresponding plantar metatarsal arteries and the layers of plantar musculature and fascia. Clear topography. Venous anatomy in the forefoot matters when symptoms do not match the usual arterial narratives. Plantar-to-dorsal communications can explain collateral drainage patterns in superficial thrombophlebitis, postoperative swelling, and in patients with chronic venous insufficiency who show prominent dorsal veins yet report plantar forefoot aching with prolonged standing. This dorsal vantage also supports procedural planning, because dorsal incisions for metatarsal osteotomies, Lisfranc exposure, or dorsal approaches to the MTP joints can disrupt superficial venous channels, while deeper plantar metatarsal veins remain paired with the neurovascular bundles that must be protected during plantar plate repair or intermetatarsal space dissection.
