Description
Across the dorsum of the male foot, superficial veins form a dorsal venous network over the metatarsals and proximal phalanges, with a prominent dorsal venous arch running transversely just distal to the ankle joint. Medially, tributaries converge toward the great saphenous vein anterior to the medial malleolus, while lateral channels course toward the small saphenous vein posterior to the lateral malleolus. Underlying bony landmarks include the phalanges, metatarsals, and dorsal aspects of the tarsals, with joint cartilage indicated at the metatarsophalangeal and interphalangeal articulations. Dorsalis pedis arterial branches and accompanying venules track between extensor tendon corridors on the dorsal midfoot. Clear surface mapping. Clinically, this dorsal view matters because dorsal pedal veins are common access points when distal peripheral IV placement is required and proximal options are limited, and their variable patterns can influence cannulation success. The dorsal venous arch also communicates with deeper plantar veins through perforators, a pathway relevant in chronic venous insufficiency and in the spread of infection or edema across the foot. Orthopedic exposures to the midfoot and forefoot, including approaches around the first metatarsal and tarsometatarsal joints, benefit from an accurate appreciation of superficial venous pathways to reduce postoperative hematoma and wound edge congestion.
