Description
Posteriorly, the pleura are rendered as a paired serous membrane investing the right and left lungs, with the visceral pleura closely applied to the pulmonary surface and the parietal pleura reflecting toward the mediastinum and diaphragm. Inferiorly, the membrane descends beyond the lung margins to form the costodiaphragmatic recess, the deepest pleural gutter at the posterolateral thorax. Superior to the lung apices, the pleural cupula rises above the first rib, while the midline airway is traced by the trachea and larynx in the superior mediastinal region. Spatial relationships read cleanly from back to front. Posterior orientation matters because pleural disease and pleural procedures are often approached with the patient sitting or leaning forward, making the posterior costodiaphragmatic recess a frequent site of dependent fluid. Small pleural effusions collect first in this recess and blunt the posterior costophrenic angle on upright radiography, and thoracentesis is commonly planned along the posterolateral chest wall to enter the pleural cavity while avoiding the inferior border of the rib where the intercostal neurovascular bundle runs. Clear recess anatomy prevents bad needle trajectories. It also clarifies why pleuritic pain localizes to parietal pleura rather than visceral pleura.
