Description
Visible in anterior aspect are the right and left lungs enclosed by pleura, with the right lung broader and the left lung smaller to accommodate the cardiac impression. Visceral pleura invests the lung surface and reflects at the hilum to become parietal pleura lining the thoracic wall, creating the pleural cavity as a potential space. Superiorly, the cervical pleura (cupula) rises above the level of the first rib toward the root of the neck, while inferiorly the parietal pleura continues onto the diaphragm as diaphragmatic pleura and onto the mediastinum as mediastinal pleura. The larynx and trachea sit in the midline, the tracheal cartilaginous rings leading inferiorly to the right and left main bronchi at the carina. Pleural anatomy is where respiratory mechanics meet procedure risk. The visceral and parietal layers slide on a thin serous film; when air enters this space, a pneumothorax can collapse the lung despite a normal bronchial tree, and when fluid accumulates, it layers dependently and blunts the costodiaphragmatic recess. This relationship underlies thoracentesis technique, needle placement just superior to the rib to avoid the intercostal neurovascular bundle, and interpretation of pleural effusion versus consolidation on imaging. A tight, teachable boundary. It also frames why apical pleura is vulnerable in subclavian central venous access and why pleuritic pain localizes to parietal pleura via intercostal and phrenic nerves.
