Description
Anteriorly oriented anatomy of the right lower limb is presented with the anterior intermuscular septum (septum intermusculare cruris anterius) emphasized as a fibrous partition between the anterior compartment and the lateral compartment of the leg. Medially, the tibial crest and anterior border of the tibia provide a hard landmark, while the fibula sits more laterally and posteriorly, giving the septum a lateral anchoring plane. Along the anterior surface, you would expect tibialis anterior, extensor hallucis longus, and extensor digitorum longus to lie anterolateral to the tibia, with fibularis (peroneus) longus and brevis positioned lateral to the septum. Clean fascial boundaries matter. Functionally, the anterior intermuscular septum helps define the closed spaces where pressure rises during acute compartment syndrome of the leg, a scenario seen after tibial shaft fractures, crush injury, or reperfusion after ischemia. The septum also frames the neurovascular course: the deep fibular (peroneal) nerve and anterior tibial artery run within the anterior compartment, while the superficial fibular nerve tracks in the lateral compartment before piercing the deep fascia distally. That spatial separation explains classic findings like foot drop from deep fibular nerve compromise versus sensory loss over the dorsum of the foot with superficial fibular nerve injury. Orthopaedic and emergency medicine educators can use this view when teaching compartment anatomy, fasciotomy planning (two-incision leg fasciotomy with release of anterior and lateral compartments), and the clinical exam landmarks that guide urgent decompression. It also fits well in surgical atlases and MSK anatomy blocks covering fascial compartments, neurovascular pathways, and the relationship of the tibia and fibula to the extensor and fibular muscle groups.
