Description
Seen from a lateral perspective in sagittal section, the septum pellucidum appears as a thin, vertical midline membrane interposed between the anterior horns of the right and left lateral ventricles. Superiorly it lies just inferior to the corpus callosum, while its inferior margin relates to the fornix and the roof of the third ventricle region. Posterior and inferior to this complex, the thalamus and hypothalamus occupy the diencephalic core, with the brainstem descending caudally and the cerebellum positioned posteroinferiorly beneath the occipital lobe. Gyri and sulci of the cerebral hemispheres frame the deep ventricular anatomy. A clean lateral/sagittal rendering is the fastest way to teach the septum pellucidum’s neighborhood, because you can read the relationships to the corpus callosum and fornix at a glance. Clinically, the septum pellucidum matters as a landmark on neuroimaging: absence or dysgenesis is associated with septo-optic dysplasia and other midline malformations, and cavum septi pellucidi or cavum vergae are common developmental variants that can be misread as cystic pathology if the surrounding ventricular anatomy is not understood. Its proximity to the anterior horns also makes it relevant when discussing hydrocephalus patterns and ventricular catheter trajectories. Small structure. Frequent point of confusion.
