Description
Running along the posterior aspect of the pancreatic head, the posterior inferior pancreaticoduodenal artery curves adjacent to the descending (second) part of the duodenum and approaches the uncinate process inferiorly. Proximally it arises from the superior mesenteric artery and ascends to meet the posterior superior pancreaticoduodenal artery, completing the posterior pancreaticoduodenal arcade. The common bile duct and portal vein lie posterior to the first part of the duodenum and pancreatic neck region, while the inferior vena cava sits deeper and slightly right of midline. Posterior visualization of the inferior pancreaticoduodenal artery matters because this vessel often provides dominant collateral flow between the superior mesenteric and celiac territories, and it is a well-known culprit in aneurysm formation when celiac axis stenosis drives increased arcade flow. It is also a named branch surgeons encounter during pancreaticoduodenectomy and during mobilization of the duodenum (Kocher maneuver), where uncontrolled back-bleeding from the posterior arcade can be mistaken for a venous source. Interventional radiologists target this artery in embolization for pancreaticoduodenal arcade aneurysms and for duodenal hemorrhage, so seeing its posterior course in relation to the pancreatic head is practical, not academic. Suitable for teaching foregut and midgut arterial anastomoses in gross anatomy and surgical anatomy courses, and for illustrating discussions of celiac trunk compression or chronic mesenteric ischemia in GI and vascular publications. It also supports operative planning and case reports in hepatopancreatobiliary surgery and endovascular therapy by clarifying the posterior arcade pathway.
