Running along the posterior aspect of the pancreatic head, the posterior inferior pancreaticoduodenal artery curves adjacent to the descending (second) part of the duodenum and approaches the uncinate process inferiorly. Proximally it arises from the superior mesenteric artery and ascends to meet the posterior superior pancreaticoduodenal artery, completing the posterior pancreaticoduodenal arcade. The common bile duct and portal vein lie posterior to the first part of the duodenum and pancreatic neck region, while the inferior vena cava sits deeper and slightly right of midline.
