Description
Deep in the suboccipital region, the rectus capitis posterior minor is visualized in sagittal section as a short, straplike muscle spanning from the posterior tubercle of the atlas (C1) to the medial part of the inferior nuchal line of the occipital bone, immediately inferior to the external occipital protuberance. Its fascicles lie posterior to the atlanto-occipital joint capsule and superior to the posterior arch of C1, with the rectus capitis posterior major positioned more laterally when included in the plane. Surrounding context in this midline cut typically includes the occipital squama, foramen magnum margin, upper cervical vertebrae with intervertebral discs, and superficial layers such as semispinalis capitis and trapezius, plus anterior landmarks like the mandible, teeth, pharynx, and suprahyoid and infrahyoid musculature. This view matters because the rectus capitis posterior minor is a key orientation point in the posterior atlanto-occipital interval, where fine dissection often becomes confusing once superficial suboccipital muscles are reflected. Many clinicians also connect this muscle to the myodural bridge, a fibrous continuity between suboccipital musculature and cervical dura that is discussed in cervicogenic headache and post-whiplash pain models, even though its clinical weight remains debated. Small muscle, big teaching value.
