Description
Posterior neck anatomy is presented with the semispinalis capitis of the transversospinalis group as the primary deep extensor running superomedially toward the occiput, deep to the trapezius and typically deep to or adjacent with the splenius capitis. Broad cranial attachment is implied along the occipital bone between the superior and inferior nuchal lines, with muscle fibers descending inferolaterally toward the cervical region where they arise from articular and transverse processes, commonly C4 to C7. Medially, the muscle borders the nuchal ligament and cervical spinous processes; laterally, it relates to the splenius capitis and the posterior triangle region as the neck transitions to the shoulder girdle. Trapezius outlines the superficial contour of the upper back. Semispinalis capitis matters because it forms the main bulk of the deep posterior neck extensors and provides a dependable landmark for understanding layered dissection from superficial splenius and trapezius down to the transversospinalis plane. This is also the muscle many clinicians palpate indirectly when evaluating cervicogenic headache patterns and extension related neck pain, and it lies in the operative corridor for posterior cervical approaches where midline and paramedian muscle splitting influences postoperative axial pain.
