Oriented from the posterior aspect of the wrist and hand, the radial collateral ligament of the carpus is shown spanning from the radial styloid process distally toward the radial margin of the carpus, along the dorsoradial border of the radiocarpal joint. Immediately ulnar to it lies the distal radius and the dorsal rim of the radiocarpal articulation, with the ulna positioned more medial (ulnar) and slightly proximal via the distal radioulnar relationship. Distal carpal landmarks expected in this projection include the scaphoid and trapezium on the radial side, continuing distally into the bases of the first and second metacarpals and the aligned phalanges. Clear bony reference points anchor the ligament’s course. Posterior visualization matters because dorsoradial wrist pain is frequently localized by palpating the radial styloid and assessing stability during ulnar deviation and axial loading across the radiocarpal joint. The radial collateral ligament helps resist excessive ulnar deviation, and it is evaluated in the setting of sprains and in patterns that overlap clinically with scaphoid injury and scapholunate dissociation after a fall on the outstretched hand. Surgeons also reference this region when planning approaches around the radial styloid and scaphoid, where protecting adjacent capsuloligamentous structures can affect postoperative stability.
