Description
Running obliquely from the spinous processes of T2 to T5 to the medial border of the scapula, the rhomboid major lies deep to the trapezius in the posterior thoracic wall of an adult male. Medially, its fibers attach along the thoracic vertebrae and the intervening supraspinous ligaments, then course inferolaterally to insert on the scapula between the level of the scapular spine and the inferior angle. Lateral to the muscle belly, the medial border of the scapula forms a hard landmark, while the middle fibers of trapezius typically overlie this region and blend toward the acromion and scapular spine. Inferiorly, the rhomboid major approaches the superior fibers of latissimus dorsi, and paraspinally the erector spinae group occupies the longitudinal gutter adjacent to the spinous processes. Posterior identification of the rhomboid major matters because scapular retraction and downward rotation are common pain generators when the periscapular muscles fatigue or when the scapulothoracic rhythm breaks down. Clinicians often confuse rhomboid strain with mid-thoracic facet pain or trapezius myofascial trigger points, so a clear map of what sits deep (rhomboids) versus superficial (trapezius) helps guide palpation, dry needling planes, and rehabilitation cueing. This is a frequent site of medial scapular border pain.
