Description
Running obliquely from the thoracic spinous processes to the medial border of the scapula, the rhomboid major lies deep to trapezius and inferior to rhomboid minor in this posterior male view. Its fibers course inferolaterally from approximately T2 to T5 toward the scapular spine region, placing the muscle medial to the scapula and lateral to the vertebral column. Trapezius is shown superficially across the posterior neck and upper thorax, framing the deeper parascapular layer. The spinous processes form the midline landmark for orientation. Seeing the rhomboid major in relation to trapezius matters when teaching scapulothoracic mechanics and when sorting out medial scapular pain. Dorsal scapular nerve irritation, often where the nerve passes through the middle scalene, can weaken rhomboid major and produce lateral drift of the scapula with reduced retraction rather than the classic serratus anterior pattern of winging. Palpation and trigger point work also depend on appreciating that trapezius must be functionally relaxed or displaced to access the rhomboid major fibers near the medial scapular border. A small muscle, a common complaint.
