Description
Medial exposure of the male thigh traces the sartorius from its proximal attachment on the anterior superior iliac spine, crossing obliquely and superficially over the anterior compartment toward its distal insertion at the pes anserinus on the superomedial tibia. Along its course, the muscle lies anterior to the hip joint and femoral shaft, then passes anteromedial to the distal femur before turning posterior to the medial femoral condyle at the knee. Superiorly, its medial border helps outline the femoral triangle, while more distally it drapes over the adductor canal as a muscular roof. Bone landmarks remain clear. Sartorius matters because its oblique path sets the surface anatomy used for femoral vascular access and regional anesthesia, where the femoral artery lies just medial to the nerve within the femoral triangle and then continues under sartorius into the adductor canal. Surgeons also reference this relationship during saphenous nerve blocks and when planning anteromedial approaches to the distal femur, since the nerve becomes vulnerable where it emerges from the adductor canal between sartorius and gracilis. Pes anserine pain is common. Distal tendinopathy and pes anserine bursitis often present with medial knee tenderness inferior to the joint line, in the same region where sartorius, gracilis, and semitendinosus converge on the tibia.
