Description
Anteriorly oriented, the sphenoid is presented with its body centered on the midline and the greater wings flaring laterally like a bat’s wings. Superior to the greater wings, the lesser wings form sharp posterior orbital margins and terminate medially as the anterior clinoid processes; between the body and lesser wings lies the optic canal. Inferior and slightly posterior, the pterygoid processes descend from the junction of body and greater wing, with medial and lateral pterygoid plates separated by the pterygoid fossa. Multiple cranial foramina are readable on the greater wing, including the superior orbital fissure at the junction with the lesser wing, and the foramina rotundum, ovale, and spinosum arranged inferolaterally. This anterior perspective is the one you want when teaching the transition from orbital apex to middle cranial fossa, because it aligns the optic canal and superior orbital fissure with the foramina that transmit V2, V3, and the middle meningeal artery. It also frames surgical corridors used in endoscopic endonasal approaches to the sella and parasellar region, where the sphenoid sinus (within the body) provides access to the pituitary while the internal carotid artery and optic nerve run immediately adjacent. Small distances matter here. Misidentifying a foramen can injure a nerve. Suitable for skull and head and neck anatomy courses, neurosurgical and ENT teaching files, and figure plates in radiology or surgical anatomy texts explaining the orbital apex, cavernous sinus region, and trigeminal nerve pathways.
