Description
Anteriorly oriented, the J shaped stomach dominates the field, with the abdominal esophagus entering at the cardia and the fundus arching superiorly under the left hemidiaphragm. The body tapers inferiorly to the antrum and pylorus, where the pyloric canal continues into the first part of the duodenum. Along the stomach’s left lateral margin, the greater curvature faces the spleen, while the pancreas lies posterior to the stomach, its body extending transversely toward the splenic hilum and its tail approaching the splenic pole. Mucosal rugae are suggested through surface contouring. An anterior view of this upper abdominal cluster is the workhorse orientation for explaining the lesser sac and why the pancreas is functionally retroperitoneal yet clinically tethered to stomach and spleen. Pancreatic acini and islets sit in a gland whose head is embraced by the duodenum, a relationship that frames the presentation of pancreatic head carcinoma with painless jaundice, versus tail lesions that can grow silently until they involve the splenic vessels. Short gastric and left gastro-omental vessels, though not always emphasized, are implied by the close apposition of greater curvature and spleen. A tight neighborhood.
