Description
Seen from a posterior perspective, the stomach (gaster) occupies the central field, with the fundus positioned superiorly and to the left and the pyloric region coursing inferiorly and to the right toward the duodenum. Tucked directly posterior to the gastric body lies the pancreas, its head anchored within the C loop of the duodenum and its tail extending laterally toward the splenic hilum. Lateral to the gastric fundus, the spleen sits in the left hypochondrium, related to the stomach by the gastrosplenic region and aligned along the long axis of the pancreatic tail. Key ductal anatomy is included, with the main pancreatic duct running longitudinally through the gland to enter the descending duodenum. Posterior relationships in this region matter because they define common operative planes and explain symptom patterns. Pancreatic inflammation or a posterior gastric ulcer can involve the pancreas because the lesser sac and the posterior wall of the stomach lie immediately anterior to the pancreatic body, and pain often refers to the back when retroperitoneal structures are irritated. This view also clarifies why lesions near the pancreatic tail can present with splenic involvement or complications near the hilum, and it provides a clean orientation for understanding the ductal route that is interrogated during ERCP and MRCP.
