Description
Perpendicular plate of the ethmoid bone forms the superior, midline component of the nasal septum, descending inferiorly from the cribriform plate while remaining continuous superiorly with the crista galli. Its thin, vertically oriented lamina is seen as a broad, irregularly curved sheet, with a sharply defined posterior border approaching the anterior face of the sphenoid (near the sphenoidal crest and rostrum) and an anterior margin that relates to the nasal bones and the nasal spine of the frontal bone. Laterally, the plate sits medial to the ethmoidal labyrinths, placing it immediately adjacent to the superior nasal cavity and the region of the superior and middle nasal meatuses. Septal relationships are easy to read. Clinically, the perpendicular plate is a frequent bony contributor to nasal septal deviation and septal spurs, often coexisting with vomerine deviation and distortion of the quadrangular cartilage, which matters in nasal obstruction workups and preoperative planning for septoplasty or endoscopic sinus surgery.
