Description
Cribriform plate (lamina cribrosa) of the ethmoid bone occupies the midline at the junction of the anterior cranial fossa and the roof of the nasal cavity, its perforated surface punctuated by multiple small foramina. A median crista galli rises superiorly from the plate, while the ethmoidal labyrinth lies laterally, helping frame the medial orbital walls and the superior nasal compartment. Along the undersurface, the plate forms the superior boundary of the nasal cavity adjacent to the superior nasal concha, while its superior surface relates to the olfactory bulbs resting on the endocranial aspect. Many tiny apertures cluster anterior to posterior, reflecting the passage of olfactory nerve filaments from the olfactory mucosa to the cranial cavity. Disruption of this thin bony partition is a classic mechanism for cerebrospinal fluid rhinorrhea after anterior skull base trauma, with associated risk of ascending meningitis. Loss of smell follows injury or shearing of olfactory fila as they traverse the foramina, and the tight anatomic relationship between cribriform plate, crista galli, and ethmoid roof is a constant point of reference in endoscopic sinus surgery and skull base approaches for encephalocele or esthesioneuroblastoma. Fine lighting and edge definition in this illustration make the sieve-like perforations readable without losing the larger landmarks needed for orientation.
