Arising from the anterior aspect of the abdominal aorta just inferior to the celiac trunk, the superior mesenteric artery (SMA) is traced in posterior view as it passes posterior to the neck of the pancreas and anterior to the uncinate process before entering the root of the mesentery. From this vantage, the vessel’s proximal segment relates closely to the superior mesenteric vein on its right, and its branches fan inferiorly and laterally toward the midgut, reaching the distal duodenum and jejunoileal loops and continuing to the right colon. The large intestine is included for orientation, with the ascending colon leading into the transverse colon and then to the descending and sigmoid colon, while epiploic appendages (appendices epiploicae) punctuate the colonic surface. Mesenteric folds suspend the bowel and carry accompanying arterial and venous arcades. Posterior emphasis on the SMA’s retropancreatic course matters in pancreatic and mesenteric surgery, where the artery forms a hard boundary for dissection around the pancreatic head and uncinate process. This is the anatomy behind both SMA syndrome (compression of the third part of the duodenum between the SMA and aorta) and the high stakes of acute mesenteric ischemia, where proximal embolus or thrombosis compromises jejunal and ileal perfusion long before colonic collaterals can compensate.
