Description
Arising within the root of the mesentery, the superior mesenteric vein ascends posterior to the small-intestinal mesentery and to the right of the superior mesenteric artery, receiving jejunal and ileal tributaries that converge from the left and right as the bowel loops coil below. Posteriorly, the vessel sits anterior to the third part of the duodenum and the uncinate process of the pancreas as it courses superiorly toward the porta hepatis region. Peripheral venules from the ileum, cecum, and ascending colon can be inferred tracking centrally within mesenteric folds, mirroring the arterial arcades but in blue. Orientation is clear. This posterior perspective matters because it matches the operative and cross sectional relationships that drive both bleeding control and iatrogenic injury risk in the inframesocolic compartment. The SMV is the key venous landmark during pancreaticoduodenectomy and right hemicolectomy, where traction on the mesenteric root can avulse thin-walled jejunal branches, and it is also central to the diagnosis of superior mesenteric vein thrombosis on contrast-enhanced CT, often associated with mesenteric ischemia or pancreatitis. Its consistent position slightly right of the SMA helps you anchor the mesenteric axis when teaching or reading imaging.
