Sural Nerve, Lateral View

Description

Running along the posterolateral calf, the sural nerve is traced from its formation in the proximal leg to its descent behind the lateral malleolus and into the lateral border of the foot. A lateral perspective places the fibula and its head laterally with the tibia more medial, while the nerve courses superficial to the deep posterior compartment and travels with the small saphenous vein toward the ankle. Proximally, the pathway relates to branches from the tibial nerve (medial sural cutaneous nerve) with contribution from the common fibular nerve via a sural communicating branch. Distally, the nerve passes posterior to the lateral malleolus adjacent to the calcaneus and lateral tarsals. This laterally oriented course matters because the sural nerve is exposed where clinicians palpate, incise, and harvest. Sensory loss or dysesthesia along the posterolateral leg and lateral foot often follows iatrogenic injury during Achilles tendon repair, lateral ankle ligament reconstruction, or percutaneous fixation around the calcaneus, and the nerve also becomes symptomatic with traction in severe ankle inversion sprains. Nerve conduction studies commonly use the sural nerve as a purely sensory target, so its predictable relationship to the lateral malleolus is a practical landmark. Small territory, big consequences.
