Description

Running along the posteromedial calf, the sural nerve is traced from its proximal formation by the medial sural cutaneous nerve (tibial nerve) with contribution from the sural communicating branch of the common fibular nerve. Distally it descends superficial to the deep posterior compartment, then courses inferior to the gastrocnemius and alongside the small saphenous vein toward the posterior aspect of the lateral malleolus. Medially, the tibia and medial malleolus provide bony reference, while the fibula lies lateral and posterior, framing the nerve’s diagonal track toward the lateral foot. Bones of the hindfoot and midfoot are included as the nerve continues to the dorsolateral border of the foot. A medial perspective clarifies the sural nerve’s relationship to landmarks used for bedside examination and operative planning, where a few millimeters matter. This is the nerve most often harvested for autologous nerve grafting and biopsied for suspected vasculitic neuropathy or amyloid, typically approached through a posterolateral ankle incision just posterior to the lateral malleolus to avoid iatrogenic injury to the small saphenous vein. Sensory territory is the key. Numbness or dysesthesia along the posterolateral calf and lateral foot after Achilles tendon repair, lateral malleolar surgery, or ankle arthroscopy frequently reflects traction, transection, or neuroma involving this superficial cutaneous nerve. Neuroanatomy and lower limb anatomy courses use this plate to teach peripheral nerve formation, named cutaneous branches, and distal sensory distributions in the calf and foot without the distraction of musculotendinous bulk.
