Description
Centered in an inferior view, the T1 vertebra presents its vertebral body anteriorly with a broad vertebral foramen posterior to it, bounded by the pedicles and laminae. Laterally, the transverse processes project from the vertebral arch, and the inferior articular processes sit posterolaterally as paired facets for the T1 to T2 zygapophysial joints. Along the posterolateral margins of the vertebral body, the inferior costal demifacets are visible as smooth articular surfaces for the head of the second rib. A key landmark. T1 matters because it sits at the cervicothoracic junction where cervical-type motion transitions to the rib-bearing thoracic spine, and that junction shapes both biomechanics and pathology. The inferior costal demifacet on T1 is a frequent teaching point in rib articulation, pairing with the superior demifacet on T2 to form the costovertebral joint for rib 2, while the tubercle of rib 2 meets the T1 transverse process at the costotransverse joint. Degenerative change or inflammatory arthropathy at these joints can refer pain to the upper posterior thorax and mimic shoulder girdle complaints, and T1 to T2 facet orientation is also relevant when planning posterior instrumentation or interpreting CT of the upper thoracic spine.
