Description
Seen from a posterior perspective, the first thoracic vertebra (T1) is centered on the vertebral arch, with the spinous process projecting inferiorly in the midline and the laminae forming the roof of the vertebral canal on either side. Lateral to the laminae, the transverse processes extend toward the costotransverse region, and the superior and inferior articular processes border the zygapophysial (facet) joints. The vertebral foramen sits anterior to the arch, framed by the pedicles and laminae. T1 also carries costal facets for the first rib, a defining thoracic feature even when the rib is not shown. T1 is a junctional vertebra: its superior articular facets and spinous process align with the lower cervical pattern, while its transverse processes and costal facets anchor thoracic cage mechanics. That mixed anatomy matters in practice. At the cervicothoracic junction, degenerative change, trauma, or postoperative alignment issues can concentrate stress at T1 to T2, and the posterior elements guide safe trajectories for instrumentation such as pedicle screws when stabilizing fractures, tumors, or deformity. A hard landmark.
