Description
Anteriorly, the T1 vertebra presents a broad vertebral body with a slightly notched superior border, flanked laterally by the pedicles that lead into the vertebral arch. Near the superolateral corners of the body, the superior costal facets (demifacets) for the head of the first rib are visible, positioned just inferior to the vertebral endplate and medial to the transverse processes. The transverse processes project laterally from the vertebral arch, while the base of the spinous process lies posterior and largely out of plane from this viewpoint. Bone texture is retained, with articular surfaces indicated by smooth, cartilage-tinted regions. T1 matters because it is the transitional vertebra between the mobile cervical spine and the rib-bearing thoracic spine, and the anatomy of its costovertebral articulation sets the pattern for upper thoracic mechanics. Small errors in identifying the superior costal facet can propagate into wrong-level localization when correlating anatomy with AP radiographs or CT, where T1 sits at the cervicothoracic junction and is often partially obscured by the shoulder girdle. A key teaching point. Clinically, the relationship of the T1 body and pedicles to the spinal canal also frames discussions of upper thoracic burst fractures and surgical planning for pedicle screw fixation in a region with narrower pedicles and limited exposure.
