Description
Spanning the posterior cervicothoracic region, the paired trapezius muscles form a broad, kite-like sheet from the external occipital protuberance and superior nuchal line down to the spinous processes of the upper thoracic vertebrae, with a midline attachment along the nuchal ligament. Laterally, the fibers converge toward the shoulder girdle, inserting along the lateral third of the clavicle, the acromion, and the spine of the scapula. Beneath the highlighted muscle you can infer the underlying bony landmarks, including the cervical and thoracic spinous processes, posterior ribs, scapular spines, and proximal humeri. Fiber direction is readable, with superior fibers coursing inferolaterally, middle fibers more transverse, and inferior fibers running superolaterally toward the scapular spine. Posterior placement makes this a practical teaching view for scapulothoracic mechanics, because the trapezius acts in coordinated force couples with serratus anterior to produce upward rotation of the scapula during overhead elevation. Weakness from spinal accessory nerve (CN XI) injury, classically after posterior triangle lymph node biopsy or neck dissection, presents with shoulder droop, impaired abduction above 90 degrees, and lateral scapular winging rather than the medial winging seen with long thoracic nerve palsy. The midline and scapular spine relationships also clarify palpation landmarks used when assessing myofascial pain patterns and postural cervicogenic headache associations in the upper trapezius.
