Description
Sectioned male kidney is rendered in a sagittal plane to expose the renal cortex peripherally and the medulla as striated renal pyramids converging toward the renal papillae. Minor calyces cup the papillae and coalesce into major calyces, which funnel medially into the renal pelvis at the renal sinus. From the inferior aspect of the pelvis, the ureter emerges as a narrow-caliber tubular duct, continuing inferiorly from the ureteropelvic junction while renal arteries and veins course centrally toward the hilum. Emphasis on the ureter leaving the collecting system matters because the ureteropelvic junction is the classic site of congenital or acquired obstruction, with resultant hydronephrosis and calyceal blunting proximal to the narrowed segment. This relationship also underpins interpretation of excretory urography and CT urography, where delayed pelvic emptying and a dilated pelvis without distal ureteral enlargement points you back to the UPJ rather than a distal ureteral calculus. Small structure, big consequence.
