Description
Anterior orientation frames the urinary system within the torso of an obese adult Black male, with both kidneys sitting high and posterior on the abdominal wall, flanking the vertebral column and tucked deep to the costal margins. Each ureter descends inferiorly from the renal pelvis, running anteromedial to the psoas region and crossing the pelvic brim before entering the urinary bladder. The bladder occupies the midline of the true pelvis just posterior to the pubic symphysis, with the urethra continuing inferiorly from the bladder neck. Obesity changes the practical anatomy even when organ positions remain recognizably standard: increased abdominal wall thickness and intraabdominal fat can make physical landmarks less reliable and alters the working depth for suprapubic bladder access, catheterization troubleshooting, and imaging correlation. This view also supports teaching the retroperitoneal location of the kidneys and ureters, a point that matters when interpreting flank pain and hematuria or when planning approaches for ureteral stent placement, percutaneous nephrostomy, or nephrectomy. Pelvic ureteral narrowing and the ureterovesical junction are natural bottlenecks. Stones commonly lodge there.
