Description

Centered within the female pelvis, the uterus sits in the midline between the urinary bladder anteriorly and the rectum posteriorly, tapering inferiorly into the cervix as it approaches the superior vagina. From the uterine cornua, the uterine (fallopian) tubes course laterally toward the ovaries, which lie on the lateral pelvic wall in the ovarian fossa, medial to the external iliac vessels and close to the ureter as it passes into the pelvis. Broad ligament reflections, the utero-ovarian ligament, and the suspensory ligament of the ovary (infundibulopelvic ligament) anchor the adnexa, while branches of the uterine and ovarian arteries track along these peritoneal folds. Surrounding context may include pelvic brim landmarks, adjacent bowel loops, and neurovascular bundles in the retroperitoneum. This configuration matters in everyday gynecologic decision-making because the adnexa share tight spatial relationships with structures that surgeons and imagers must protect, most memorably the ureter running inferior to the uterine artery (water under the bridge) during hysterectomy and uterine artery ligation. Ovarian torsion pivots on the suspensory ligament and utero-ovarian ligament, so seeing how the ovary hangs from its vascular pedicle clarifies why torsion can rapidly compromise the ovarian vessels. Ectopic pregnancy, tubo-ovarian abscess, and endometriosis also map predictably onto the tube, ovary, and posterior cul-de-sac.
