Description
Paired ovaries occupy the lateral pelvic cavity within the adnexa, positioned medial to the pelvic sidewall and inferior to the pelvic brim, with their long axes oriented roughly anteroposterior. Each ovary lies adjacent to the fimbriae of the uterine tube, with the mesovarium attaching along the anterior (hilar) border where the ovarian vessels and nerves enter, while the free posterior surface faces the rectouterine region. Surrounding context in the lower torso cutaway logically includes coils of small intestine and sigmoid colon superiorly, the pelvic floor musculature inferiorly, and regional vasculature and lymphatics coursing along the iliac vessels. Nerve branches track through the retroperitoneum into the pelvis and toward the proximal lower limb. Clear separation of systems helps. An adnexal-focused composition matters because ovarian pathology often presents as a mass effect in a crowded pelvis, and surgical decision-making depends on knowing what sits lateral and posterior to the ovary, and what can be safely mobilized. The ovarian artery typically arises from the abdominal aorta and travels in the suspensory ligament of the ovary to the hilum, a pedicle that becomes central during oophorectomy or salpingo-oophorectomy, while lymphatic drainage follows these vessels to para-aortic (lumbar) nodes rather than the pelvic nodal basins many learners expect. Pelvic pain patterns also make more sense when you see autonomic fibers and plexuses running with the gonadal vessels in the retroperitoneum.
