Description
Ovoid in contour, the ovary (ovarium) is presented as a gross specimen emphasizing its outer covering, with the germinal epithelium forming the superficial layer over the dense, whitish tunica albuginea. Along the medial border, the hilum represents the entry point for ovarian vessels, lymphatics, and autonomic nerves as they pass through the mesovarium, while the free lateral surface remains peritoneal. Superiorly and posteriorly, the attachment to the suspensory ligament of the ovary (infundibulopelvic ligament) is inferred by the vascular pedicle, and the uterine pole points inferomedially toward the proper ovarian ligament. Surface anatomy matters here. That superficial architecture has direct clinical implications: ovarian surface epithelium (historically termed germinal epithelium) is the presumed site of origin for many high grade serous and related epithelial ovarian cancers, a teaching point when discussing why early disease often remains occult and why peritoneal spread is common. The tunica albuginea also frames the concept of ovulatory rupture and subsequent repair, and it provides a clean gross landmark when correlating with cortical follicles and functional cysts on pathology specimens.
