Seen from the external surface, the ovary (ovarium) appears as an almond-shaped gonad suspended along the lateral pelvic wall, positioned inferior to the pelvic brim and lateral to the uterus within the ovarian fossa. Medially, its uterine pole faces the uterine tube and uterine cornu, while the tubal pole points superiorly toward the infundibulum and fimbriae. A mesenteric attachment at the anterior border, the mesovarium, tethers the ovary to the broad ligament, and peritoneal folds extend toward the uterus as the ovarian ligament and toward the pelvic sidewall as the suspensory ligament of the ovary (infundibulopelvic ligament) conveying the ovarian vessels. That suspended relationship matters in the operating room and on cross sectional imaging because the ovarian blood supply travels in the infundibulopelvic ligament, not through the uterus, so vascular control during oophorectomy targets the lateral pedicle near the pelvic brim. Torsion follows the same anatomy: rotation around the vascular pedicle compromises venous outflow first, and the ovary often migrates medially and superiorly as the utero-ovarian ligament and mesovarium twist. Normal peritoneal coverings also explain why ovulation releases an oocyte directly into the peritoneal cavity before fimbrial capture. Small structure, high stakes.
