Ovoid ovarian tissue (ovarium) is presented with its anterior and posterior surfaces distinguished, bounded superiorly and inferiorly by the tubal and uterine poles. Along the medial aspect, the mesovarian border (hilum) marks the attachment to the broad ligament and the entry point for the ovarian vessels and nerves, while the free lateral border faces the pelvic sidewall. A subtle surface contour suggests the cortex beneath the tunica albuginea and the deeper medulla, the typical organization of the female gonad in gross anatomy. Orientation of the ovary matters because most clinical and surgical conversations hinge on poles and attachments, not on abstract “sides.” The tubal pole relates to the fimbriae of the uterine tube and the suspensory ligament of the ovary (infundibulopelvic ligament), a common site of vascular control during adnexal surgery, while the uterine pole connects via the proper ovarian ligament to the uterus near the uterine cornu. Misidentifying these landmarks can complicate oophorectomy, ovarian cystectomy, or torsion assessment, where the pedicle around the hilum and suspensory ligament becomes the axis of twisting.
