Description
Ovarian gross anatomy is presented with emphasis on the outer cortex and the central medulla (vascular core). The tunica albuginea forms a thin fibrous layer immediately deep to the surface covering, while the cortex occupies the peripheral, more superficial zone and surrounds the medulla centrally. Medullary stroma contains the larger ovarian vessels and nerves entering at the hilum along the mesovarian attachment, so the most vascular tissue sits deep and medial compared with the follicle-bearing cortex. Clear compartmentalization. That cortex to medulla relationship matters when you are thinking about ovarian torsion, where twisting of the infundibulopelvic ligament and utero-ovarian ligament compromises venous outflow first, causing stromal edema and ultimately arterial ischemia concentrated in the medullary core. It also frames surgical planning for ovarian cystectomy, where staying superficial to the medulla helps preserve functional cortical tissue and reduces bleeding from hilar vessels. Pathologists and radiologists use this same architecture when correlating gross sectioning with ultrasound and MRI, since a thickened, echogenic cortex with peripheral follicles contrasts with a congested, enlarged central stroma in torsion.
