Description
Running along the anteromedial femoral shaft, the vastus medialis forms the medial portion of the quadriceps femoris, with fibers arising from the medial lip of the linea aspera and intertrochanteric line and descending toward the superomedial patella. From a medial perspective, its distal oblique bundle (vastus medialis obliquus) is seen converging on the quadriceps tendon and medial patellar retinaculum, lying anterior to the adductor magnus and medial intermuscular septum and deep to the sartorius as it approaches the knee. Superiorly, the muscle belly abuts the hip region musculature at the proximal thigh, while inferiorly it narrows toward the patellofemoral joint line. Patellar alignment is the story here. Clinically, this angle highlights why loss of vastus medialis tone or delayed activation can contribute to lateral patellar tracking and anterior knee pain, a common thread in patellofemoral pain syndrome and after periods of immobilization. The medial viewpoint also helps you appreciate the relationship of the distal vastus medialis to the adductor canal region, a consideration during medial knee approaches, total knee arthroplasty exposure, and when planning rehabilitation that targets terminal knee extension without aggravating the patellofemoral joint.
