Description
Running along the posterolateral pelvic floor, the vesical venous plexus forms a dense network of small veins and venules around the inferior and lateral surfaces of the urinary bladder, shown in profile relative to the pubic symphysis anteriorly and the rectum posteriorly. Tributaries course along the bladder base toward the region of the prostate and pelvic fascia, then converge to drain into the internal iliac venous system in continuity with the inferior vena cava superiorly. In the same lateral field, loops of jejunum and ileum lie superior and anterior to the pelvis, while the sigmoid colon and rectum descend posteriorly toward the anal canal. Bony landmarks from the lumbar spine and pelvis anchor the vascular relationships. For pelvic surgeons, this plexus is where clean planes stop being clean. Venous channels around the bladder neck and prostatic base can bleed diffusely during retropubic prostatectomy, cystoprostatectomy, or transvesical approaches because the plexus lacks a single dominant trunk and communicates with adjacent prostatic and pudendal veins. The lateral perspective also helps teach how pelvic venous congestion and valveless communications contribute to brisk back-bleeding when the bladder is mobilized off the endopelvic fascia. Expect variability.
