Description
Positioned on the palmar (volar) aspect of the hand, the volar plates appear as dense fibrocartilaginous sheets spanning the anterior surfaces of the metacarpophalangeal and interphalangeal joints. Each plate lies immediately deep to the flexor tendons and their synovial sheaths, blending proximally with the joint capsule and attaching distally to the base of the adjacent phalanx while the proper collateral ligaments course along the medial and lateral margins of the joint. Proximal in the palm, the metacarpals and phalanges provide the osseous framework, with the plates forming a thickened anterior restraint in front of each joint line. Clean anatomy. Palmar views of the volar plate matter because hyperextension injuries fail here, and the pattern of failure helps you distinguish a simple sprain from a true volar plate avulsion at the base of the middle phalanx, a common mechanism at the proximal interphalangeal joint after a ball strike. In chronic settings, laxity or scarring of the volar plate contributes to PIP hyperextension and can progress toward swan-neck deformity, so the exact relationship between plate, capsule, and collateral ligaments guides both splinting strategy and operative repair.
