Description
Running obliquely from the zygomatic bone (malar eminence) to the modiolus at the oral commissure, the paired zygomaticus major muscles are emphasized bilaterally on the male face in an anterior (frontal) view. Each belly lies superficial to the buccinator and deep to the subcutaneous tissues of the cheek, coursing inferomedially from the lateral midface toward the corner of the mouth. Surrounding landmarks include orbicularis oculi encircling the palpebral fissure superiorly and orbicularis oris forming the sphincter of the lips, with adjacent perioral elevators and depressors suggested by the layered gray musculature of the midface and neck. Clinically, the zygomaticus major is the prime mover of the smile, and its relationship to the modiolus explains why perioral scarring, cleft lip repair, or trauma near the oral commissure can blunt smile symmetry despite otherwise intact facial tone. Facial nerve anatomy sits close to the surface here. Buccal and zygomatic branches of the facial nerve (CN VII) traverse the cheek and are vulnerable during parotid surgery, facelift dissection, and repair of malar lacerations, where iatrogenic denervation can produce an asymmetric grin that is often more apparent than weakness elsewhere.
