Description

Seen in a segment of large intestine, the colonic wall is rendered with mucosa and submucosa ballooning outward as multiple diverticula, forming sac-like pouches that project beyond the muscularis propria. Most outpouchings arise on the mesenteric side where the vasa recta pierce the circular muscle, so the diverticula cluster between the taeniae coli rather than along them. The serosal surface retains its haustral contour, while the luminal lining stays smooth, underscoring that the process is a structural herniation rather than a primary mucosal ulceration. No inflammation is suggested. Diverticulosis matters because the same weak points that allow a pouch to form also permit bleeding when an associated penetrating artery is exposed, a classic source of brisk lower gastrointestinal hemorrhage without pain. The distribution also tracks clinical reality: diverticula concentrate in the sigmoid colon where intraluminal pressure peaks, which is why left lower quadrant tenderness and pericolic fat stranding on CT are the usual pattern when diverticulitis develops. Surgeons and endoscopists use these landmarks to anticipate where perforation, fistula formation, and segmental thickening are most likely, and to explain why symptoms can range from none at all to acute peritonitis.
