Description
Cut through a male kidney, the outer renal cortex forms a continuous peripheral rim, while the renal medulla lies deep to it as a series of triangular renal pyramids with their apices (papillae) directed medially toward the renal pelvis. Between adjacent pyramids, cortical tissue extends inward as renal columns, separating the medullary cones and maintaining continuity with the superficial cortex. Medial to the pyramids, the funnel-shaped renal pelvis occupies the renal sinus and receives urine via the minor and major calyces before it exits into the proximal ureter. Stylized nephron elements run radially through the pyramids, consistent with the course of loops of Henle and collecting ducts toward the papilla. That corticomedullary arrangement is the anatomic basis for urine concentration and for many exam-relevant correlations. Papillary tips are where collecting ducts empty, so papillary necrosis (classically associated with analgesic nephropathy, diabetes mellitus, sickle cell disease, and acute pyelonephritis) localizes to this medullary zone and can be taught cleanly against the pyramid geometry. The view also clarifies why medullary ischemia occurs readily, the inner medulla operates at low oxygen tension, and how the corticomedullary junction becomes a landmark in reflux nephropathy and scarring patterns.
