Description
Sagittal cross-section of an adult male kidney exposes the renal cortex as the outer parenchymal rim and the renal medulla arranged into multiple renal pyramids whose bases face the cortex and whose apices converge toward the renal sinus. Each papilla projects into a minor calyx, and the minor calyces unite into major calyces that drain to the renal pelvis, which continues inferiorly as the ureter. Corticomedullary junction landmarks are clear, with medullary rays extending from cortex toward the pyramid bases and the collecting system centered medially in the hilum region. Renal columns (cortical tissue) separate adjacent pyramids. Emphasizing the pyramids and papillae supports teaching the urine concentrating mechanism and the path anatomy follows from collecting ducts to calyces and pelvis. Papillary tips are where sloughed tissue or mineral deposits present clinically, as in renal papillary necrosis from analgesic nephropathy, diabetes mellitus, or sickle cell disease, and where small calculi can lodge to trigger hematuria and colicky pain as they enter the calyceal system. For endourology and imaging, this sectional anatomy maps directly to CT urography and to calyceal access planning in percutaneous nephrolithotomy, where selecting an appropriate posterior calyx aims to minimize vascular injury.
