Description
Oblique lateral viewing of the heart emphasizes the superficial coronary venous network coursing within epicardial fat over the atria and ventricles, with blue venous channels tracking alongside red coronary arteries in the atrioventricular and interventricular grooves. The great cardiac vein ascends in the anterior interventricular sulcus toward the left atrioventricular groove, while smaller cardiac veins and venules spread laterally over the left ventricular free wall and around the base near the left atrium. Posteriorly directed tributaries converge toward the coronary sinus in the posterior coronary sulcus, anatomically positioned inferior to the left atrium and superior to the posterior left ventricle. A short segment of the great vessels is also included, with the ascending aorta and arch providing a superior anterior landmark. Coronary venous anatomy matters when you need a mental map for interventions that depend on the coronary sinus system rather than the coronary arteries. Cardiac resynchronization therapy commonly places a left ventricular pacing lead through the coronary sinus into a lateral or posterolateral cardiac vein, and an unfavorable venous angle, small caliber branch, or prominent Thebesian valve can limit lead delivery. Venous drainage patterns also influence interpretation of coronary sinus catheterization and retrograde cardioplegia during cardiac surgery. Orientation is the point here.
